COVID-19 Vaccine Incentive

All faculty, staff, and students who become or are already fully vaccinated (both doses of Pfizer or Moderna or one dose of Johnson and Johnson) may enter a drawing for chances to win prizes below. Only one entry and one prize awarded per fully-vaccinated person. Only currently enrolled students or all full-time, part-time, temporary, and rehired-retiree employees can receive incentives. Individuals will submit their name and contact information via an online form link below. Those who have already submitted the form do not have to do it again. Everyone must fill the form out to be entered-even those who were vaccinated at the Student Health Center.

Prize winners should be fully vaccinated (meaning have received both shots of Pfizer or Moderna, or a single shot of the J&J vaccine) at least one day prior to drawing date and will be required to show proof to take possession of the prize.

The Food and Drug Administration (FDA) announced on August 23, 2021 that the Pfizer COVID-19 vaccine is now fully approved for those ages 16 and older.
